Three weeks into our travel baseball season, one of my best 11-year-old outfielders walked into practice with a hitch in his stride. He’d played in back-to-back tournaments the weekend before — Saturday through Sunday, five games total — and he hadn’t said a word to me or his parents about his hip hurting. He’d been feeling it since Friday night. He just didn’t want to miss a start.
We caught it before it became a stress fracture. Barely.
Youth coaches are the first line of defense against sports injuries — not team doctors, not athletic trainers, not parents. Most youth programs don’t have medical staff at practice. They have you. And the data is clear: more than half of all youth sports injuries are preventable. The difference almost always comes down to whether the coach has a system before an athlete hits the ground.
This guide is built around what we’ve learned over two decades on the field — not a medical textbook, but a practical playbook you can run at practice, across any sport, starting this week.
The Real Injury Problem Youth Coaches Face
Youth sports injuries have risen sharply over the past two decades, driven by two converging trends: early single-sport specialization and year-round play. Kids are competing more months of the year with less rest — and often in a single sport starting at age eight or nine.
The result: overuse injuries now account for roughly half of all youth sports injuries. These aren’t traumatic collisions — they’re slow-building stress responses that accumulate over weeks or months. By the time an athlete says something hurts, the injury is often well established.
The second problem is harder to solve: athletes who hide pain. In our experience coaching baseball, soccer, and basketball from 7U through high school, the athletes most likely to conceal an injury are also the most driven — the ones you count on most. They’ve learned, from the culture around them, that mentioning pain means sitting out.
Your job as a coach is to catch both: the physical patterns that predict injury before it happens, and the team culture where every athlete knows that reporting pain is always the right call. The 5-Minute Pre-Practice Movement Screen
Most injury prevention resources tell coaches to “use a proper warm-up.” That’s not wrong — it’s just not specific enough. What we’ve built over years of coaching is a brief movement screen embedded in the first five minutes of every practice. It requires no equipment, takes one coach to observe, and consistently flags the athletes who are hurting but haven’t said anything yet.
Run these five movements as a group. You’re watching for limping, asymmetry, pain faces, or an athlete who bails out of the pattern. Anyone who triggers a flag gets a quiet one-on-one check before full activity begins.
This screen does not replace medical evaluation. What it does is put information in your hands early enough to ask the right question: “Hey — I noticed you were favoring your left side on the shuffles. How’s that feeling?” That question, asked before practice is over, has prevented more injuries in our program than anything on this list.
Overuse Injuries: Recognize the Signals Early
Because overuse injuries develop gradually, they telegraph warning signs coaches can read before the injury becomes serious:
- Localized tenderness an athlete can point to with one finger, not general muscle soreness
- Pain that worsens during activity rather than loosening up once they get moving
- Swelling around a growth plate area — knee, ankle, elbow, or shoulder
- A persistent limp or guarded movement the athlete hasn’t mentioned
- An athlete who protects a joint — short-arming a throw, favoring one leg on cuts, rolling the foot differently
Growth plates are especially vulnerable between ages 9 and 14. In throwing sports, the medial epicondyle growth plate in the elbow is the most common source of early arm damage — which is exactly why pitch count limits exist and why we enforce them regardless of playoff implications. In soccer and basketball, Osgood-Schlatter at the knee and Sever’s disease at the heel are extremely common and frequently dismissed as “growing pains.”

The most effective single prevention tool a coach controls is mandatory rest. Not suggested rest — mandated. In our program, every athlete gets at least one full day off from organized sport per week, and we enforce pitch count limits even when a parent is in the stands waving us off. We’ve had arguments. We’ve held the line.
Multi-sport participation is the other lever coaches can actually pull. Athletes who play multiple sports distribute stress across different movement patterns and muscle groups, significantly reducing overuse risk. Concussion Basics Every Coach Needs
The CDC’s HEADS UP program is the standard, and if you haven’t completed their free online concussion training for youth coaches, do that before your next practice. Here’s the field version every coach needs to know cold.
Remove the athlete immediately if you see any of these:
- Headache or pressure in the head
- Nausea or vomiting
- Balance problems or dizziness
- Double or blurry vision
- Sensitivity to light or noise
- Seems foggy, confused, or slow to respond
- Doesn’t remember the play or the contact
The rule is simple: when in doubt, sit them out. There is no monitoring period on the sideline. Returning a concussed athlete to play is the single most dangerous decision a coach can make. A second impact while the brain is still recovering from the first can cause permanent damage or death.
In our program, any athlete who comes off for a suspected concussion does not return to practice or competition until we have written clearance from a physician or licensed healthcare provider. No parent can override this. No game situation changes it. We put this in our pre-season parent communication — using a letter like our parent communication template — so the expectation is established before we’re standing on a field trying to have the conversation.
Building a Culture Where Athletes Report Pain
This is the gap almost no published injury prevention resource addresses directly — and it’s the one that matters most in practice.
Athletes hide pain for two reasons: they don’t want to miss playing time, and they’ve absorbed a culture where playing through pain is praised. Coaches, parents, and youth sports culture at large have built this dynamic unintentionally. Coaches are the only people positioned to dismantle it.
A few things we do specifically:
We never praise playing through pain. “Tough it out” is gone from our coaching vocabulary. When we see an athlete competing hard through obvious discomfort, we call a timeout and check in privately. Reporting pain is what our team does — we say this out loud, to the whole team, early in the season.
We use exit checks. At the end of every practice, as athletes grab water, we do a quick verbal round: “How are you feeling physically — anything to tell us about?” Giving athletes a specific moment and a direct question removes the social pressure of volunteering information unprompted. Simple. It works.
We brief parents once per season. Athletes who hide injuries will not receive more playing time for it. We make this explicit, not implied.
Smart Warm-Up Design
A warm-up means dynamic movement preparation — not two laps around the field. Research consistently shows that static stretching before activity (holding a stretch for 30+ seconds) reduces power and sprint speed output for 30–40 minutes afterward. Save it for post-practice.
Our standard dynamic warm-up for any sport runs 8–10 minutes: high knees and butt kicks, walking lunges with a hip opener, A-skips and B-skips, lateral shuffles and carioca, arm circles and shoulder circles for throwing sports, then a few sport-specific activation movements — jump stops, first-step cuts, or swings — before the first drill.
A well-designed warm-up also builds the bodyweight movement patterns that protect joints over an entire season. Pairing this with dedicated bodyweight exercises for young athletes a few times per week builds the baseline strength that keeps athletes resilient.
Heat, Hydration, and Rest
Athletes under 14 regulate body temperature less efficiently than adults — they produce more heat per unit of body mass and sweat less effectively. Hot-weather practice requires active management, not just a water jug on the bench.
Our hot-weather protocol: water break every 20 minutes when temperatures exceed 80°F (not “when kids seem thirsty,” since thirst lags dehydration by 15–20 minutes); modified practice intensity when the heat index hits 90°F; any athlete who arrives to practice dehydrated gets modified activity for the first 20 minutes; and any athlete who reports dizziness, stops sweating on a hot day, or seems disoriented comes off the field immediately.
When to Hold vs. Return to Play
The default answer is always conservative. General muscle soreness after a hard practice? Fine to push through. Localized joint pain, growth plate tenderness, head trauma, or anything with a specific onset? Hold.
Our three-question field test before returning any athlete:
- Can they perform the sport movement without compensating or guarding?
- Has the pain been absent for at least 48 hours at rest?
- For anything beyond mild soreness: do we have clearance from a parent or healthcare provider?
If any answer is no, the athlete does not return to full activity that day. This policy has cost us a few heated conversations with parents. To our knowledge, it has not cost any athlete a preventable long-term injury.
